COMPLETE IN ORDER TO BE ELIGIBLE TO RECEIVE PAYMENT FOR THE RETURN OF CERTAIN CHARGES FOR THE REQUEST OF MEDICAL RECORDS
Your signed, completed Proof of Claim must be submitted to the Settlement Administrator electronically no later than November 30, 2026.
You will see a list of eligible transactions to select from during the claim filing process.
